Registration

We will be happy to send our registration forms to you. If you are covered by dental insurance, additional information such as group number, employer name and the address to send the insurance form, will be needed. Social security numbers for you and your spouse are required on most insurance forms. Also, the date of birth of the patient and spouse. If the patient is a student going to college- the name of the college is needed.
